Cancellation Policy
If it is necessary to miss an appointment, please call the office at least 24 hours in advance.
There is a $60 fee for missed appointments or those cancelled within 24 hours of the scheduled time.
Keep in mind that follow-up appointments are scheduled at intervals considered necessary for good clinical care. Thus, it might not be possible to continue prescribing medications (i.e. authorizing refills) for people who have missed appointments.

Payment is required at the time services are rendered. Patients are responsible for copayment or coinsurance if our office is in-network. Patients with non-participating or no insurance will be required to pay in full on the date of service. Our office will supply you with a receipt that can be submitted to your out-of-network insurance company for reimbursement.
Initial consultations are $345.00; medication management appointments are $120.00.
