We have made these forms available to you so you can complete them and bring them with you to your initial visit.
If you complete your forms in advance we ask that you arrive, 10 minutes early to allow us time to process your paperwork. If you do not complete the forms in advance, we ask that you arrive 20 minutes prior to your scheduled time.
If your forms are not completed prior to your appointment time you will be required to reschedule.
In addition, you MUST have your insurance card with you on the day of your visit or you will be required to reschedule.
